A brain injury can change many aspects of daily life, but with the right support, individuals can work toward greater independence and confidence. Therapy services can play an important role in helping people develop strategies, strengthen skills, and navigate everyday activities after a brain injury.
At Minds Matter, LLC, we provide home and community-based therapy services designed to meet individuals where they are, literally and figuratively. Our team works with people who have experienced a traumatic brain injury (TBI) or acquired brain injury (ABI) and offers five types of therapy: occupational, physical, speech, cognitive, and behavioral therapy.
Understanding Brain Injury
A traumatic brain injury occurs when an outside force causes damage to the brain. Examples include a brain injury resulting from a motor vehicle accident or a fall.
An acquired brain injury occurs after birth and is caused by an internal event or medical condition rather than an external force. Examples include a stroke or a brain tumor.
Regardless of how a brain injury occurred, every person’s recovery and support needs are different. Therapy can help individuals work toward goals that are meaningful to their daily lives, whether that means improving mobility, communication, memory, problem-solving, emotional regulation, or the ability to complete everyday activities.
How to Begin Services With Minds Matter
Getting started is as simple as submitting a referral. Anyone can submit a referral, and a referral can be made at any point following a brain injury. The referral may come from the individual, a family member or friend, medical professional, case manager, or another person involved in the individual’s support.
A referral can be submitted by:
- Phone: 913-789-9900
- Email: referrals@mindsmatterllc.com
- Online: Minds Matter’s brain injury therapy referral form
- Fax
The only information required to begin the referral process is the individual’s name, date of birth, and county of residence. Additional information, particularly medical history and current needs, can be extremely helpful to our team as they prepare for the initial meeting.
Because Minds Matter provides services primarily in individuals’ homes and communities, we will also ask for information about where the person will be staying for intake and therapy services.
What Information Will You Need to Provide?
Our Referrals Team will ask for some basic information to help us understand the individual’s circumstances and determine the best way to move forward. This may include:
- First and last name
- Date of birth
- County of residence
- Service address and mailing address, if different
- Best contact person and phone number
- General information about the brain injury
- Current insurance coverage
- How you heard about Minds Matter or who referred you
You do not need to share information about the injury that you are uncomfortable disclosing. Our goal is to gather enough information to help our team begin the process—not to make the referral process overwhelming.
Understanding Insurance and Coverage
Minds Matter primarily provides services to individuals who receive coverage through a brain injury waiver through state Medicaid. We also work with a number of private and commercial insurance plans and offer services for individuals who are paying out of pocket.
If insurance coverage is not yet in place, our team may be able to assist with the process of pursuing coverage. Depending on the individual’s circumstances, Minds Matter can also provide assistance with paperwork related to Medicaid services and Social Security Disability Insurance (SSDI).
What Happens After a Referral?
Once a referral is submitted, a Minds Matter Intake Coordinator will typically contact you within 72 hours to schedule an intake meeting. The Intake Coordinator assigned to you will depend on the county where the individual resides.
During the intake meeting, the team will learn more about the individual’s goals, current needs, therapy preferences, benefits, and available supports. This information helps our team determine how therapy services can best support the individual’s journey toward greater independence.
A brain injury can bring unexpected challenges, but you do not have to navigate the process alone. Taking the first step by submitting a referral can connect you with a team focused on understanding your goals and helping you move forward.
If you or someone you know has experienced a brain injury and could benefit from home and community-based therapy services, Minds Matter is here to help.
